When you contact our office, we will direct you to complete the Travel Consultation Intake Form. Once received and reviewed by our staff, we will proceed with booking a Travel appointment with our nurse practitioner, Antonia Evans, FNP-C.
A full travel visit includes CDC travel guidance and recommendations on vaccines and medications if needed. Including education about why they are recommended and how to properly take them. The visit also includes education about food and water safety, insect bite protection and preventing/treating common illnesses. Please contact our office six weeks prior to travel. Some patients will need vaccines a few week prior to travel.
Our practice does not include a travel appointment with any other type of appointment. The travel appointment is available for current patients with a recent well visit in our practice. The travel appointment is prepaid one time only fee of $120. If we carry the vaccines, this appointment fee will also include the vaccines given. Some of the vaccines may not be in our office and may require additional fees. In line with current American Academy of Pediatrics Guidelines and medical standard of care, our practice does not prescribe medication or vaccines over the phone without an appointment.
